Acknowledgement and Agreement of Waiver, Release and Participation Conditions

Vault Lacrosse Skills Camp — June 3–5, 2026

This form sets out the legal rights and responsibilities with respect to your (and your minor child(ren)'s) participation in, or otherwise attendance at, any given Vault Sports, Inc. ("Vault") lacrosse training session or event to be conducted at any time commencing on June 3, 2026 and running through June 5, 2026 (each, an "Event"), and is governed by laws of the state of Florida irrespective of conflict of laws rules. If you do not understand the responsibilities and waiver / release of claims as herein outlined, you are strongly encouraged to consult an attorney or otherwise to not permit your minor child(ren) to participate in the Event.

REQUIREMENT OF SIGNED ACKNOWLEDGEMENT AND AGREEMENT. No Event invitee or proposed Event participant will be permitted to actually participate in an Event unless a properly completed and signed Acknowledgement and Agreement for the applicable child(ren) is in place and received by Vault prior to commencement of the applicable Event. Once signed by the applicable parent/guardian, this Acknowledgement and Agreement will remain in effect and will apply to any and all Events the referenced child(ren) may attend or participate in, during the Event period referenced above.

NO REFUND OF EVENT FEES PAID. Vault is required to advance various amounts to secure the Event facilities, as well as to ensure the participation of Event coaches, trainers, instructors, referees, and celebrity personnel. In addition, Vault will expend various amounts in preparation of each Event, including expenditures to purchase necessary Event equipment and novelty items / gear to be provided to Event participants. Accordingly, Event registration is not subject to cancellation, and paid Event participation fees are non-refundable. Your space at any given Event is not guaranteed until the Event participation fee has been paid, and once paid, no refunds of Event participation fees will be provided. In the event that any given Event is cancelled due to lightning or extraordinary weather or other conditions that could adversely affect the quality of the Event or make the Event unsafe (at Vault's discretion), then a make-up date will be scheduled, and the paid Event participation fees will be applied to the make-up Event.

WAIVER AND RELEASE OF CLAIMS. As consideration for participation in the Event, I, for myself and the below-referenced minor(s), which I specifically represent I have the full right and authority to enter into this document on behalf of, hereby WAIVE any claim to which I or he/she/they may become entitled for any loss, damage or injury to personal property or injury (including death), and RELEASE Vault, its now or hereafter affiliated companies, their advertising and promotional agencies, the owners and managers of the Event field or facility, and any of their employees, agents, personnel, service providers, instructors, trainers, coaches, celebrity personnel, referees, on-site staff, suppliers and contractors assisting with the Event (collectively referred to herein as the "Vault Team") from liability for any injury or damage caused by or attributable to, either directly or indirectly, any act or omission of the Vault Team.

ASSUMPTION OF RISK. I further state and represent that I am of the full age of majority and am competent and authorized to make any determination respecting my child(ren)'s participation in the Event. I am aware that lacrosse is a contact sport, and that participation involves risk of personal, physical and/or mental injury as well as damage to personal property. I agree to assume all risk for all injuries to my child(ren), myself and any others that I may bring to the Event.

OUTDOOR CONDITIONS. I acknowledge that the Event may be conducted outdoors and that unless there is lightning in the area, the Vault Team may determine to move forward with the Event as scheduled in spite of any weather conditions.

PUBLICITY RELEASE AND CONSENT. I hereby give my full, irrevocable permission for the Vault Team to freely use my and/or my child(ren)'s name, likeness and photographic image(s) in connection with promotions or publicity of the Event or hereafter Vault events, activities or offerings, without compensation of any kind.

ACKNOWLEDGEMENT OF PARENT / GUARDIAN OF PARTICIPANT

By checking the box and entering your full legal name during online registration, the undersigned parent or guardian acknowledges that he or she has read this document in its entirety, understands it, and agrees on behalf of himself/herself and the below-referenced minor(s) to be bound by its terms and conditions. This electronic acknowledgement constitutes a legally binding agreement under the laws of the State of Florida.
